Community Residential Settings:
- Our CRS waiver homes offer housing for 4 individuals with disabilities who each live in their own bedroom and get some services from on-site caregivers. These settings are for people who need some daily care, but do not need “skilled nursing” care by definition.

Cypress, Chisholm – 4 resident capacity (1 opening available)
Cypress is a 4 bed waiver home nestled in a quiet residential neighborhood in Chisholm, near the elementary school and within walking distance to Longyear Lake. The home is a modern two story house, with a spacious split level main floor. There are two private bedrooms on each floor decorated to individual preferences. At Cypress, dynamic individuals have the opportunity to participate in person-centered planning and enjoy a variety of opportunities in the community based on the residents’ preference. Family involvement is encouraged to promote individual growth. The home has 24 hour awake staff supervision, and in-person nurse consultation services.
Logan – 4 resident capacity (2 openings available)
Logan is a 4 bed waiver home located in a residential neighborhood in Buhl. The individuals living at Logan enjoy private bedrooms decorated to their preferences, bright and open living areas, and a back patio. Logan provides 24-hour awake staff supervision, and in-person nurse consultation services. Family involvement is recognized as a valuable part of our residents’ lives. Individuality and independence is promoted through a variety of community integration opportunities, as well as person-centered programming.
