Adult Foster Care (AFC) / Community Residential Settings:
- Our AFC/CRS waiver homes offer housing for 4 individuals with disabilities who each live in their own bedroom and get some services from on-site caregivers. AFC is for people who need some daily care, but do not need “skilled nursing” care by definition. Adult Foster Care is sometimes called Community Residential Settings (CRS).

Cypress, Chisholm – 4 resident capacity (1 openings available)
Cypress is a 4 bed waiver home nestled in a quiet residential neighborhood in Chisholm, near the elementary school and within walking distance to Longyear Lake. The home is a modern two story house, with a spacious split level main floor. There are two private bedrooms on each floor decorated to individual preferences. At Cypress, dynamic individuals have the opportunity to participate in person-centered planning and enjoy a variety of opportunities in the community based on the residents’ preference. Family involvement is encouraged to promote individual growth. The home has 24 hour awake staff supervision, and in-person nurse consultation services.
Mapleview, Hibbing – 4 resident capacity (1 openings available)
Mapleview is a 4 bed waiver home located in a residential neighborhood in Hibbing. Mapleview is conveniently located near many shops, churches, restaurants, and the movie theater. This home is a spacious one level home with attached garage, lots of storage, and extra wide doorways and hallways for enhanced accessibility. The individuals living at Mapleview enjoy private bedrooms decorated to their preferences, bright and open living areas, a front patio, and a wooded scenic backyard with views from the living room and deck. Individuality and independence is promoted through a variety of community integration opportunities, as well as person-centered programming. Mapleview provides 24-hour awake staff supervision, and in-person nurse consultation services. Family involvement is recognized as a valuable part of our residents’ lives.
